General Information
To ensure faster service please make sure each and every line of the Client Intake forms are filled out completely before sending to Administrative Bankruptcy Associates for processing, please do the following:
- Make certain the entire set contains all 28 pages;
- Make certain client has filled out the forms completely;
- Avoid using “?” as answers;
- Ensure that questions that are answered with a "yes" on the Statement of Affairs have a detailed answer along with the question;
- Compare the Income Form to an actual debtor paycheck stub to confirm the amounts are accurate. Please provide a copy of current paycheck stub;
- To verify debtor’s identity obtain a copy his/her driver's license and social security card and submit with Client Intake form;
- Please include with the Client Intake forms all copies of court pleadings pertaining to any pending lawsuits, foreclosures, or wage garnishments; finally
